SOME OF THE HIGHEST NEED CLIENTS GET LOST AND FALL THROUGH THE CRACKS UPON RELEASE. YOU CAN IMAGINE -- AND SORRY. SKIPPING AROUND.
YOU CAN IMAGINE HOW HARD IT IS TO TAKE CARE OF YOUR CANCER, YOUR MENTAL HEALTH DIAGNOSIS, YOUR HIV, WHILE YOU'RE ALSO SCRAMBLING FOR AN ID OR FOR FOOD, FOR WORK, FOR TRANSPORTATION, FOR HOUSING. I SKIPPED THE HIGH NEEDS PART WHICH IS THAT ROUGHLY 70% OF OUR FOLKS IN BEXAR COUNTY JAIL HAVE SUBSTANCE-RELATED ISSUES AND ABOUT 25% ARE ALREADY ON PSYCHOTROPIC MEDICATIONS. SO WE DO SEE THAT PEOPLE, IF THEY DON'T HAVE SUPPORT AFTER RELEASE, ARE MORE COMMONLY -- ARE MORE PRONE TO HAVE HEALTH PROBLEMS AND IN FACT, A 2007 RESEARCH STUDY REPORTED THAT IN THE FIRST TWO WEEKS AFTER RELEASE, THE RISK OF DEATH IS 12.7 TIMES HIGHER THAN IN THE GENERAL POPULATION.
ACCIDENTAL OVERDOSE IS A REASON, HEART ATTACK, SUICIDE, HOMICIDE, CANCER. COMMUNITY HEALTH WORKERS ARE A WAY TO SUPPORT SUCCESSFUL RE-ENTRY INTO SOCIETY. WE'VE MENTIONED COMMUNITY HEALTH WORKERS AT OTHER PRESENTATIONS. I WANT TO GO DEEPER TODAY. THEY'RE ALSO CALLED PROMOTORAS. IT'S SOMEONE WHO IS COMING FROM THE POPULATION THEY ARE SERVING, TRUSTED MEMBER, OR THEY HAVE AN UNUSUALLY CLOSE UNDERSTANDING OF THE COMMUNITY AND WHETHER THEY'RE WORKING FOR METRO HEALTH OR NOT, WE CONSIDER THEM FOR FRONT LINE HEALTH WORKERS.
BECAUSE THE INTERPLAY BETWEEN THESE RESOURCES, THEY ALSO END UP IMPROVING THE QUALITY AND THE CULTURAL COMPETENCE OF THE SERVICE DELIVERY. THERE IS A STRONG EVIDENCE-BASED -- THIS BULLET POINT IS REFERRING TO A LARGE RANDOMIZED CONTROL TRIAL SHOWING THE RETURN ON INVESTMENT IS $2.47 SAVED PER DOLLAR SPENT.
TAKING A STEP BACK TO THE CHW HUB THAT CLAUDE MENTIONED, SO THIS IS NOT WHERE ALL OF THE CHWS AT METRO HEALTH WORK. THAT'S A COMMON MISUNDERSTANDING. WE DO PROVIDE CONTINUING EDUCATION FOR CHWS, FOR CHW INSTRUCTORS, SCHOLARSHIPS TO GET YOUR CERTIFICATION. WE ORGANIZE THE ANNUAL CHW SUMMIT. THE ASSESSMENT OF ACCESS TO CARE DURING COVID-19, THE REPRODUCT INVENTORY JUSTICE FUND, AND THEN THE CARE INITIATIVE WILL BE PROVIDE PROVIDERS OF QUALITY AND AFFORDABLE AND RESPECTFUL HEALTH CARE HERE IN BEXAR COUNTY. MOVING BACK TO UNLOCKED, WE HAVE A TEAM OF COMMUNITY HEALTH WORKERS PROVIDING QUALITY RESOURCES, AND THAT MEANS THESE ARE VETTED RESOURCES. WE'RE NOT JUST HANDING SOMEBODY A PHONE NUMBER AND A PIECE OF PAPER. WE'RE LOOKING FOR AGENCIES THAT EMPOWER AND RESPECT PEOPLE. THAT HELPS OUR CLIENTS TRUST US, AND IT ALSO EMPOWERS THE CLIENTS TO BE SUCCESSFUL IN THEIR REINTEGRATION INTO THE COMMUNITY. I'VE ALLUDED TO THE PRIORITY MAY NOT BE MEDICAL CARE WHEN THEY ARE COMING OUT. THE CHW MODEL SUPPORTS PROVIDING COACHING AND ADVOCACY AND NAVIGATION FOR THE CLIENT'S OWN ACTION PLAN WITH THE IDEA THAT IF WE CAN HELP PEOPLE GET TO THEIR FEET SOONER THAN HOPEFULLY THEY GET TO THEIR HEALTH GOALS SOONER MANY IT'S DIFFERENT FROM, SAY, A PATIENT NAVIGATOR. UNLOCKED SPECIFICALLY SERVES PEOPLE WHO WERE RELEASED IN THE LAST THREE MONTHS FROM BEXAR COUNTY JAIL, AND BETWEEN THE TIME WE SUBMITTED THIS SLIDE AND NOW, I CAN SAY THAT WE ARE NOW SERVING 120 CLIENTS. I'VE MENTIONED THE CLIENT-CENTERED AND INDIVIDUALIZED REFERRAL PLAN, THE QUALITY SERVICES. MOST RECENTLY, THE HIGHEST NEEDS IDENTIFIED BY OUR CLIENTS WERE EMPLOYMENT, LEGAL ASSISTANCE, FOOD, AND MENTAL HEALTH. IN THE MOST RECENT GRANT YEAR, WE PROVIDED 98 CLOSED LOOP REFERRALS, AND WE'RE ON TRACT TO SERVE 150 CLIENTS THIS YEAR. I SHOULD ALSO MENTION THIS PROGRAM DID NOT EXIST BEFORE 2022 AND THE FIRST ONE AND A HALF YEARS WERE SPENT CREATING THOSE -- VETTING THOSE REFERRAL SOURCES, CREATING RELATIONSHIPS, CREATING MOUS, CITY AND COUNTY CONTRACTS, TALKING TO JUSTICE INVOLVED PEOPLE WHAT THEY WANTED IN A PROGRAM LIKE THIS AND ALSO HIRING THE RIGHT TEAM, WHICH WE NOW HAVE.I WOULD SAY THIS IS OUR FIRST REAL YEAR OF OPERATIONS. OUR TEAM IS DOING GREAT, AND I WOULD LOVE TO SHARE SOME -- A COUPLE OF SUCCESS STORIES WITH YOU. SO WE HAVE A, WHO WAS SIX MONTHS PREGNANT WHEN WE MET HER, AND WAS SCARED TO RELAPSE THROUGH DRUG USE. IT IS REALLY, REALLY COMMON IN OUR SYPHILIS CASES TO SEE THAT PEOPLE HAD NO PRENATAL CARE AND WERE USING SUBSTANCES. THOSE THINGS OFTEN GO HAND IN HAND. BUT WITH UNLOCKED SENATOR, A HAS -- SUPPORT, SHE HAS A A BABY GIRL NOW AND HAS OFFERED TO BECOME A MENTOR WHEN SHE COMPLETES HER PROGRAM, AND SHE'S RECONNECTED WITH HER FAMILY AND CHILDREN. THAT'S ONE GREAT EXAMPLE. ALSO, CLIENT B, A RETIRED NAVY VETERAN, UNLOCKED HELPED HIM SECURE HOUSING, GAIN EMPLOYMENT, CLEAR HIS OUTSTANDING TICKETS, REPAIR RELATIONSHIPS WITH HIS FAMILY MEMBERS AS WELL, AND REGAIN HIS DRIVER'S LICENSE, WHICH IS PUTTING HIM ON TRACK TO BECOME A CDL DRIVER. I HAVE TO THANK OUR PARTNERS, UNIVERSITY HEALTH HELPED FLAG HIGH NEED PATIENTS FOR US BECAUSE THEY'RE THE PROVIDER IN THE JAIL. THE RE-ENTRY CENTER IS WHERE WE DO OUR WORK. HE WALKED DOWN AND STAYED THERE UNTIL 8:00 WHEN IT OPENED TO GET SERVICES FROM THE UNLOCKED TEAM. SO THE RE-ENTRY TECHNICIANS ON THE OUTSIDE OF THE JAIL PROVIDE SUPPORT TO US. THE STAFF WAS JUST THERE ON THURSDAY, AND WE HAD WALK-INS. WHEN PEOPLE LEAVE JAIL AND THEY WALK DOWN THE STREET, THEY'RE LOOKING AND THEY'RE HUNGRY AND WILLING TO PARTICIPATE. SO PART OF THAT ASSESSMENT AND BEING ABLE TO ENGAGE THE CLIENTS IS LETTING THEM KNOW THAT THIS IS THEIR PROGRAM. SO WHATEVER IS IMPORTANT TO THEM IS IMPORTANT TO US. I'M GIVING YOU A LITTLE BIT OF BACKGROUND. I THINK SOME OF YOU ARE ACTUALLY ON THE CCR. YOU MIGHT RECALL, THIS WAS SUBMITTED BACK IN NOVEMBER BY COUNCILMEMBER MCKEE-RODRIGUEZ. IT WAS REQUESTED FOR CITY STAFF TO ENGAGE IN A MASTER PLANNING EFFORT FOR OUR HISTORIC CITY CEMETERIES TO INCLUDE ELIGIBLE FUNDING TO SUPPORT THE EXECUTION OF THE MASTER PLAN RECOMMENDATIONS. THIS ITEM WENT TO GOVERNANCE ON MARCH 20TH AND WAS REFERRED YOU TO AT THE COMMITTEE LEVEL. JUST A LITTLE BIT OF EXTRA TIED TO ■THATCCR,THE REQUEST WAS MEANT TO UPLIFT AND BEAUTIFY THE CITY CEMETERIES. THEY SIT ON THE EAST SIDE. THEY ARE SURROUNDED BY FOUR MAJOR STREETS. SO IT'S REALLY UPDATE ITS LOOK, PAY RESPECT TO THE HISTORY OF THE CEMETERIES, AND TO INCREASE THE CONNECTION WITH THE SURROUNDING COMMUNITIES.
SO THAT'S WHAT WE WOULD BE LOOKING FOR IN A MASTER PLAN. GIVING YOU FAST FACTS IF YOU'RE NOT TOO FAMILIAR, THE STREETS THAT SURROUND OUR CEMETERIES, NORTH NEW BRAWN FELLS, COMMERCE, EAST HOUSTON, AND HACKBERRY. THE CEMETERIES WERE FOUNDED IN 1853. IT'S 103 ACRES. THERE'S 33 CEMETERIES TOTAL. 16 ARE PRIVATELY OWNED.
THERE ARE 9 CITY MAINTAINED CEMETERIES. THEY ARE SHADED IN THE DARK GREEN YOU SEE THERE. IN 1901, THE CITY DID BECOME THE STEWARDS OF THESE 9 HISTORIC CEMETERIES. THEY'RE IN THE DARK GREEN. WE NOTED THE NAME OF THE OWNERSHIP OF THE OTHERS. SO INTERESTING TO NOTE, THERE IS ALREADY AN EXISTING MASTER PLAN FOR THE CITY CEMETERIES. IT WAS DONE IN 1990. SO I'LL GIVE YOU BACKGROUND. IN 1987, THERE WAS THE EAST SIDE SAN ANTONIO ECONOMIC DEVELOPMENT COUNCIL. THEY SPONSORED A STUDY. THERE WERE THREE MAJOR RECOMMENDATIONS OF THIS STUDY. ONE WAS TO CONDUCT A SURVEY AND SUBMIT A NOMINATION TO THE NATIONAL REGISTRY OF HISTORIC PLACES. TWO, TO ESTABLISH AN EAST SIDE CEMETERY FOUNDATION AND THE FOUNDATION WAS MEANT TO FUND RAISE, INCREASE AWARENESS, WORK ON MAINTENANCE AND ADMINISTRATION. THREE WAS TO ACTUALLY PREPARE A MASTER PLAN. SO THAT IS WHAT CAME OUT OF THAT FEASIBILITY STUDY IN 1987. SO IN 1990, THE CITY UTILIZED GRANT FUNDS TO CONDUCT THIS MASTER PLAN USING AN EXTERNAL TEAM OF CONSULTANTS. WHAT THE RECOMMENDATIONS -- THE COMPONENTS OF THE MASTER PLAN -- ACTUALLY, THEY DID HISTORICAL -- THERE'S A LOT OF HISTORICAL IN THERE OF HOW THE CEMETERIES CAME TO BE AND WHAT THE CURRENT USAGE LOOKED LIKE. THEN THERE WERE A SERIES OF RECOMMENDATIONS TIED TO LAND USE, PLAN DESIGN, MAINTENANCE, THE ESTABLISHMENT OF A VISITOR'S CENTER AND A CHAPEL ON SITE, CAPITAL IMPROVEMENT, COST ESTIMATES, AND FUNDING REQUIREMENTS. THEY LOOKED AT THE LEGAL ASPECTS AS WELL. IT WAS A COMPREHENSIVE MASTER PLAN THAT WAS DONE. WE WA WANTED TO NOTE, IN 2000, WE WERE ADDED TO THE HISTORIC PLACES. WE ACCOMPLISHED TWO OF THE THREE RECOMMENDATIONS FROM THE FEASIBILITY STUDY.
CURRENT MAINTENANCE OF THE CITY CEMETERIES, WE DO HAVE A PARK STEWARDSHIP DIVISION, AND THEY OVERSEE THE MAINTENANCE OF THE CEMETERIES TO INCLUDE MOWING, TREE CARE, LITTER PICK UP, AND THEY MONITOR FOR VANDALISM AS WELL. WANTED TO HIGHLIGHT A COUPLE OF PROGRAMS THAT WE DO IN ADDITION TO MAINTENANCE. WE HAVE A CITY CEMETERY STEWARDSHIP PROGRAM. IT'S AVAILABLE FOR THOSE FOLKS AGES 15 AND UP WHO WANT TO TAKE AN ACTIVE ROLE IN PRESERVATION AND BEAUTIFICATION OF THESE

THEY MEET UP ON THE SECOND SATURDAYS OF EVERY MONTH. WE DO HAVE ONE COMING UP MAY 11TH. WHAT THEY DO, WE ACTUALLY PARTNER WITH THE OFFICE OF HISTORIC PRESERVATION, AND THERE IS A VIDEO PRESENTATION THAT HAS TO BE WATCHED BEFORE THEY CAN PARTICIPATE. SO, YOU KNOW, YOU HAVE ABOUT 300 ARTISTS, MAYBE WHEN WE GET THE AIRPORT ONLINE, YOU KNOW, 50, 60 ARTISTS. HOWEVER, WHAT WE'VE DONE IS WE DO LOOK AT HAVING COLLABORATIONS.
