BILLING CONTRACT. SAFD. >> GOOD MORNING.
MY NAME IS ABIGAIL AND I WAS THE AUDIT MANAGER FOR THIS PROJECT.
I WILL BE PRESENTING THE AUDIT RESULTS OVER THE FIRE DEPARTMENT EMS BILLING CONTRACT. OVERALL OUR OBJECTIVE WAS TO DETERMINE IF THE FIRE

DEPARTMENT WAS ADEQUATELY MANAGING THE BILLING CONTRACT AND WE DID DETERMINE THAT FIRE HAD ADEQUATE PROCEDURES FOR BOTH THE COLLECTIONS RATE AND THE AMOUNT THOSE PAID TO THE CONTRACTOR BMP. ADDITIONALLY WE CONFIRMED THAT FIRE HAD ADEQUATE CONTROLS OVER A CONTRACTUAL REQUIREMENTS SUCH AS SBEDA GOALS, MONTHLY REPORTING AND THE PROTECTION OF PROTECTED HEALTH INFORMATION BY SUBCONTRACTORS. HOWEVER, WE DID DETERMINE THAT CONTROLS COULD BE IMPROVED OVER BILLING ACCURACY, SYSTEM ACCESS AND SUBCONTRACTOR INSURANCE COVERAGE. AS OUR FIRST RECOMMENDATION WAS OVERBILLING ACCURACIES, TO PROVIDE SOME CONTEXT REGARDING THE WORK THAT OUR TEAM PERFORMED, WE ANALYZED SIX MONTHS WORTH OF DATA. SO FOR THIS TEST WE LOOKED AT BOTH THE PATIENT CARE FILE AS WELL AS THE BILLING FILE AND OVERALL WE DETERMINED THAT SERVICES WERE NOT ACCURATELY BILLED. ON THE SLIDE YOU'LL SEE THE FIVE INSTANCES THAT WE IDENTIFIED.TI THE FIRST OF WHICH WAS THAT ALL MEDICATIONS ADMINISTERED WERE NOT SUBJECT TO BILLING AND TO EXPAND ON THIS POINT WE DETERMINED THAT THIS WAS DUE TO THE FACT THAT THERE WERE NOT -- THAT THEY WERE NOT RATES ESTABLISHED FOR ALL MEDICATIONS, THEREFORE THEY COULD NOT BE BILLED. ADDITIONALLY WE DETERMINED THAT THE MEDICATION EPINEPHRINE WAS NOT CONSISTENTLY BILLED AND THIS HAD TO DO IN DIFFERENCES IN THE NAMING CONVENTION AS SET UP IN THE SYSTEM.WE ALSO DETERMINED THAT ALL DOSAGES ADMINISTERED WERE NOT BILLED.
TO PROVIDE AN EXAMPLE HERE, THERE WAS A SYSTEM ERROR. SO IF THERE WERE THREE DOSAGES OF EPINEPHRINE ADMINISTERED, ONLY ONE WAS BEING BILLED.
WE ALSO DETERMINED THAT NON-TRANSPORT AID ONLY SERVICES WERE NOT CONSISTENTLY ASSESSED. SO FOR EXAMPLE, IF TRANSPORTATION DIDN'T APPLY, BUT THE PATIENT DID RECEIVE AID ONLY, THEIR VITALS WERE TAKEN, A 100-DOLLAR FEE SHOULD HAVE BEEN ASSESSED AND IN SOME INSTANCES IT WAS NOT.
AND LASTLY, TRANSPORTATION SERVICES, WE ALSO IDENTIFIED INSTANCES WHERE THESE WERE NOT SUBJECT TO THE 1,000-DOLLAR FEE THAT APPLIED AT THE TIME OF OUR AUDIT. SO AS A RESULT WE MADE A FEW RECOMMENDATIONS IN THIS AREA. THE FIRST WAS THAT THE APPROVED MEDICATIONS LIST BE APPROVED AND UPDATED AS NECESSARY. WE ALSO RECOMMENDED THAT FIREWORK WITH THE SOUTH TEXAS REGIONAL ADVISORY COUNCIL.
THEY ARE ACTUALLY THE SYSTEM OWNERS OF IMAGE TREND, WHICH IS THE PATIENT DATA CARE SYSTEM, TO SEE IF THERE'S A POSSIBILITY TO INCORPORATE SYSTEM APPLICATION CONTROLS TO TRY TO PREVENT SOME OF THESE ISSUES ON THE FRONT END.
AND LASTLY, WE RECOMMENDED THAT THE FIRE DEPARTMENT REVIEW THE RESULTS OF OUR DATA ANALYSIS FURTHER AS WELL AS DEVELOP MONITORING CONTROLS TO IDENTIFY ISSUES SUCH AS THESE MOVING FORWARD FORWARD. THE NEXT SLIDE COVERS OUR FINAL TWO RECOMMENDATIONS, THE FIRST WAS FOR IMAGE TREND USER ACCESS.
AS I PREVIOUSLY MENTIONED, THIS IS THE ELECTRONIC PATIENT REPORTING SYSTEM THAT IS UTILIZED BY EMS. WE DETERMINED THAT ACCESS WAS NOT APPROPRIATE.
ADDITIONALLY WE DETERMINED THAT SUBCONTRACTOR INSURANCE COVERAGE DID NOT MEET CONTRACTUAL REQUIREMENTS. SIMILAR RECOMMENDATION FOR THESE TWO ITEMS WAS THAT CONTROLS BE MODIFIED TO ENSURE THAT BOTH USER ACCESS IS ADEQUATE MOVING FORWARD AND THAT SUBCONTRACTORS HAVE THE REQUIRED INSURANCE COVERAGE.
THE FIRE DEPARTMENT AGREED WITH OUR RECOMMENDATIONS AND HAS SINCE DEVELOPED A CORRECTIVE ACTION PLAN WITH AN ANTICIPATED COMPLETION DATE OF OCTOBER 2025.

THE OBJECTIVE OF THE AUDIT WAS TO DETERMINE IF CRT OPERATIONS FOR BOTH SAPD AND METRO HEALTH WERE IN COMPLIANCE WITH PROGRAM POLICIES AND PROCEDURES AND IF THE REPORTING OF METRICS WAS ACCURATE. TO PROVIDE A LITTLE MORE CONTEXT ON CRT AND THE TYPES OF CASES THAT THEY WORK WITH, THEY WORK WITH VICTIMS OF DOMESTIC VIOLENCE CASES WHERE THE CASES ARE CLASSIFIED AS MISDEMEANORS AND THE SUSPECT -- AND THEY ARE INVOLVED IN AN INTIMATE PARTNER RELATIONSHIP AND/OR THEY ARE A PARENT AND AN ADULT CHILD LIVING IN THE SAME RESIDENCE.
SO THOSE ARE THE TYPES OF CASES THAT THEY WORK WITH. OUR OVERALL CONCLUSION IS THAT BOTH SAPD AND METRO HEALTH ARE ACCURATELY REPORTING CRT METRICS.
IN ADDITION, SAPD IS PROPERLY MAINTAINING SUPPLEMENTARY DOCUMENTATION FOR CASES IN MARK 53. THEY ARE BEING APPROPRIATELY CATEGORIZED AS MISDEMEANOR OR FELONY. HOWEVER THEY ARE NOT DONE ACCORDING TO PROGRAM POLICY AND PROCEDURES. SO OUR FIRST RECOMMENDATION IS FOR BOTH SAPD AND METRO HEALTH. WE REVIEWED TRAINING RECORDS FOR ALL CRT MEMBERS FROM BOTH DEPARTMENTS AND WE DETERMINED THAT NOT ALL CRT MEMBERS HAVE COMPLETED THEIR REQUIRED TRAINING FOR THE DOMESTIC VIOLENCE PROGRAMS. THE CRT MEMBERS DEPENDING ON THEIR POSITION IN THE TEAM REQUIRE TRAINING THAT THEY NEED TO COMPLETE.
OUR RECOMMENDATION WAS TO ENSURE THE EXISTING CONTROLS ARE DONE AS INTEND AND ALSO PERIODIC REVIEW OF TRAINING. OUR NEXT RECOMMENDATION WAS FOR SAPD IN THE INITIAL DANGER ASSESSMENTS. WE REVIEWED THESE CASES AGAIN IN MARK 43 AND WE DETERMINED THE LINGER ASSESSMENT FOR LAW ENFORCEMENT FORMS ARE NOT CONSISTENTLY COMPLETED FOR VICTIMS. THIS FORM KNOWN AS THE DALE FORM IS A CHECKLIST THAT USES AN INITIAL THREAT ASSESSMENT FOR THE VICTIM.

COMPLETING A SECONDARY DANGER ASSESSMENT FORM.AND ALSO PROPERLY SECURING AND DISBURSING TANGIBLE GOODS TO VICTIMS. OUR RECOMMENDATION WAS TO REVISE THE EXISTING PROCEDURES TO PROVIDE CLEAR GUIDANCE AND TIME FRAMES FOR THE CRT TASKS. AND THE LAST ONE INVOLVED INAPPROPRIATE ACCESS. WE VISITED THE SUBSTATIONS WHERE CRT MEMBERS WORK AND WE DETERMINED THAT PHYSICAL ACCESS TO THE KEYS THAT OPEN SAFES IS INAPPROPRIATE AND USER ACCESS TO CASE FILES CONTAINING PII, WHICH IS PERSONALLY IDENTIFIABLE INFORMATION, AND PHI, WHICH IS PROTECTED HEALTH INFORMATION, IS INAPPROPRIATE. SO I THINK THAT'S -- I'D LIKE TO SEE IT FOR DATA, BUT I THINK IF WE DID A DEEPER DIVE WE WOULD SEE THE INITIAL DANGER ASSESSMENT TO WHAT IS DONE BY CRT IN FOLLOW-UP OR COULD BE TWO TOTALLY DIFFERENT THINGS. AND THAT'S WHY I AM NOT SURE, AND CLAUDE, I DON'T KNOW IF YOUR TEAM CAN LOOK AT IN TERMS OF THE CRISIS THIS REALLY IS -- FOR THOSE OF YOU WHO AREN'T FAMILIAR -- A TRIAGE UNIT.

THEY HAVE ALL THESE CASES AND THEY HAVE TO KIND OF TRIAGE IN TERMS OF WHAT THEY THINK IS GOING TO -- WHAT'S GOING TO BE THE CRISIS THAT HAPPENS NEXT.AND IN SOME CASES IT COULD BE THAT SIMPLE HARASSMENT CASE WHERE HE'S CALLED 20 TIMES AND WON'T LEAVE HER ALONE. AND THOSE ARE THE STORIES WE HEAR. THOSE ARE THE STORIES OF VICTIMS THAT THEIR FAMILIES HAVE TO START FOUNDATIONS BECAUSE IT WENT FROM 0 TO 60 LIKE THAT.
AND THOSE ARE THE ONES THAT ARE THE MOST DIFFICULT TO DO THAT.
SO I AM GLAD THAT YOU GUYS ARE MAKING THE CORRECTIVE ACTIONS.
I THINK WE'RE GOING TO NEED AN UPDATE EITHER AT PUBLIC SAFETY COMMITTEE OR MAYBE IN A B SESSION ON HOW THIS TREND IS -- YOU KNOW, NOT JUST WHAT WE'VE DONE AS A RESULT OF THIS AUDIT, BUT ALSO WHAT WE NEED TO DO TO KIND OF HAVE MORE -- LOOK AT MORE TRAINING. KIND OF LIKE COUNCILMAN COURAGE HAS SAID IS DO WE NEED MORE TRAINING? DO WE NEED MORE FEATURES? DO YOU NEED MORE STAFF? DO WE NEED TO, YOU KNOW, HAVE CRISIS RESPONSE TEAM LEVEL 1, CRISIS TEAM LEVEL RESPONSE LEVEL 2? WHAT DO WE NEED TO DO TO BE BETTER AT THIS.
AND I THINK THAT THAT IS ONE THING THAT THROUGH COUNCILMAN COURAGE, WE NEED TO LOOK AT THE STRUCTURE AND MAYBE WE DO A MORE -- WE LOOK AT GIVING DESIGNATIONS TO PATROL OFFICERS THAT JUST DO A LITTLE MORE, KIND OF LIKE WE DO WITH THE MENTAL HEALTH UNIT WHERE THEY HAVE THAT EXTRA TRAINING, MAYBE WE DO THAT WITH PATROL THAT HAVE THAT LITTLE EXTRA TRAINING ON HOW TO TALK TO MILLENNIALS, HOW TO TALK TO SENIORS, ELDER ABUSE. I KNOW THERE'S OPPORTUNITIES OUT THERE AND MAYBE IF WE TALK THROUGH THE BUDGET CYCLE WE CAN DO A LITTLE BIT MORE THAN THAT.
